    Laparoscopic totally extraperitoneal inguinal hernia repair in the elderly: A prospective control study

    Inguinal hernia (IH) repair can be obtained with both open and laparoscopic techniques, which are usually performed using a transabdominal preperitoneal (TAPP) or a totally extraperitoneal (TEP) approach. The aim of the study was to evaluate whether the results of laparoscopic TEP IH repair in the elderly ( 6565 years old) are different with respect to results obtained in younger patients. One hundred and four consecutive patients (four women and 100 men, median age of 57 years, range=21-85 years) with unilateral (N=21, 20.2%) or bilateral (N=83, 79.8%) IH were prospectively enrolled in the study. Patients were divided into two groups according to their age: group A (N=68, 65.4%) aged <65 years and group B (N=36, 34.6%) aged 6565 years. The mean operative time was not significantly different between groups (48\ub120 vs. 52\ub120 min, p=0.33). One case of increased PaCO2 was observed in each group (p=0.72) and two and one case of pneumoperitoneum (p=0.57) in groups A and B, respectively. Two (1.9%) patients (one in each group; p=0.55) required TEP conversion. Mild postoperative complications developed in four patients of each group (p=0.44). After one-year follow-up, three (2.9%) recurrences occurred (group 1=1, group 2=2, p=0.55), both in patients who had undergone direct IH repair. The overall postoperative relative risk of complications related to age was 1.08 (95% confidence interval=0.91-1.27, p=0.53). In conclusion, our results suggest that in patients with IH scheduled for TEP repair, age does not represent a contraindication to surgery in terms of complication rate and postoperative results

    Post-AGB Stars in Globular Clusters and Galactic Halos

    We discuss three aspects of post-AGB (PAGB) stars in old populations. (1) HST photometry of the nucleus of the planetary nebula (PN) K 648 in the globular cluster (GC) M15 implies a mass of 0.60 Msun, in contrast to the mean masses of white dwarfs in GCs of ~0.5 Msun. This suggests that K 648 is descended from a merged binary, and we infer that single Pop II stars do not produce visible PNe. (2) Yellow PAGB stars are the visually brightest stars in old populations (Mv ~ -3.3) and are easily recognizable because of their large Balmer jumps; thus they show great promise as a Pop II standard candle. Two yellow PAGB stars in the GC NGC 5986 have the same V magnitudes to within +/-0.05 mag, supporting an expected narrow luminosity function. (3) Using CCD photometry and a u filter lying below the Balmer jump, we have detected yellow PAGB stars in the halo of M31 and in its dwarf elliptical companion NGC 205. With the Milky Way zero point, we reproduce the Cepheid distance to M31, and find that NGC 205 is ~100 kpc further away than M31. The star counts imply a yellow PAGB lifetime of about 25,000 yr, and their luminosities imply masses near 0.53 Msun.Comment: 6 pages, 2 figures.     BACKGROUND: Endometriosis is defined as an inflammatory condition of the female reproductive tract, a state often associated with infertility and miscarriage. Many exogenously administered factors (treatments) control the disease via as yet unknown pathways. Possible candidate molecules involved in these mechanisms could be the serum-soluble human leukocyte antigens (sHIA) that have been detected in a variety of human body fluids and that are associated with several diseases. AIMS: We here examine how danazol and leuprorelin acetate depot treatments exert their anti-inflammatory action. It is plausible that subtle alterations mediated by these treatments and in relation to sHLA may explain the pathophysiology of endometriosis and provide insights towards new therapeutic protocols. METHODS: Indirect enzyme-linked immunosorbent assay (ELISA), using specific monoclonal antibodies, determined serum-soluble class-I and class-II HLA levels. ELISA readings from treated women were compared with normal healthy subjects. RESULTS: Serum-soluble class-I and class-II HLA levels are statistically significantly lower (P < 0.001) in women with endometriosis than in the control groups. However, danazol but not leuprorelin acetate depot administration augments soluble HLA class I and class II (P < 0.01 and P < 0.001, respectively) to normal levels during the treatment period, an increase that may account for the anti-inflammatory effect and the remission observed. CONCLUSIONS: It is shown that one of the underlying causes of endometriosis may be the lack of both circulating class-I and class-II antigen levels. Danazol administration acts via an induced release of these antigens, whose presence correlates with the degree of the inflammatory alleviation obtained. We thus provide evidence that the inflammatory state of the disease appears to be associated with soluble HLA levels because, 3 months after ceasing therapy, the circulating antigens in the serum return to the same levels that correspond to the pathological condition

    Orbital eccentricities of binary systems with a former AGB star

    Many binary stellar systems in which the primary star is beyond the asymptotic giant branch (AGB) evolutionary phase show significant orbital eccentricities whereas current binary interaction models predict their orbits to be circularised. We analyse how the orbital parameters in a system are modified under mass loss and mass exchange among its binary components and propose a model for enhanced mass-loss from the AGB star due to tidal interaction with its companion, which allows a smooth transition between the wind and Roche-lobe overflow mass-loss regimes. We explicitly follow its effect along the orbit on the change of eccentricity and orbital semi-major axis, as well as the effect of accretion by the companion. We calculate timescales for the variation of these orbital parameters and compare them to the tidal circularisation timescale. We find that in many cases, due to the enhanced mass loss of the AGB component at orbital phases closer to the periastron, the net eccentricity growth rate in one orbit is comparable to the rate of tidal circularisation. We show that with this eccentricity enhancing mechanism it is possible to reproduce the orbital period and eccentricity of the Sirius system, which under the standard assumptions of binary interaction is expected to be circularised. We also show that this mechanism may provide an explanation for the eccentricities of most barium star systems, which are expected to be circularised due to tidal dissipation. By proposing a tidally enhanced model of mass loss from AGB stars we find a mechanism which efficiently works against the tidal circularisation of the orbit, which explains the significant eccentricities observed in binary systems containing a white dwarf and a less evolved companion, such as Sirius and systems with barium stars.Comment: 9 pages, 5 figures, accepted for publication in Astronomy and Astrophysics on 24th of October of 200

    Variability and spectral classification of LMC giants: results from DENIS and EROS

    We present the first cross-identifications of sources in the near-infrared DENIS survey and in the micro-lensing EROS survey in a field of about 0.5 square degrees around the optical center (OC) of the Large Magellanic Cloud. We analyze the photometric history of these stars in the EROS data base and obtain light-curves for about 800 variables. Most of the stars are long period variables (Miras and Semi-Regulars), a few Cepheids are also present. We also present new spectroscopic data on 126 asymptotic giant branch stars in the OC field, 30 previously known and 96 newly discovered by the DENIS survey. The visible spectra are used to assign a carbon- (C-) or oxygen-rich (O-rich) nature to the observed stars on the basis of the presence of molecular bands of TiO, VO, CN, C2. For the remaining of the stars we used the (J-Ks) color to determine whether they are O-rich or C-rich. Plotting Log(period) versus Ks we find three very distinct period-luminosity relations, mainly populated by Semi-Regular of type a (SRa), b (SRb) and Mira variables. Carbon-rich stars occupy mostly the upper part of these relations.     Spectral index of the H2O-maser emitting planetary nebula IRAS 17347-3139

    We present radio continuum observations of the planetary nebula (PN) IRAS 17347-3139 (one of the only two known to harbour water maser emission), made to derive its spectral index and the turnover frequency of the emission. The spectrum of the source rises in the whole frequency range sampled, from 2.4 to 24.9 GHz, although the spectral index seems to decrease at the highest frequencies (0.79+-0.04 between 4.3 and 8.9 GHz, and 0.64+-0.06 between 16.1 and 24.9 GHz). This suggests a turnover frequency around 20 GHz (which is unusual among PNe, whose radio emission usually becomes optically thin at frequencies < 10 GHz), and a relatively high emission measure (1.5 x 10^9 cm^{-6} pc). The radio continuum emission has increased by a factor of ~1.26 at 8.4 GHz in 13 years, which can be explained as expansion of the ionized region by a factor of ~1.12 in radius with a dynamical age of ~120 yr and at an expansion velocity of ~5-40 km/s. These radio continuum characteristics, together with the presence of water maser emission and a strong optical extinction suggest that IRAS 17347-3139 is one of the youngest PNe known, with a relatively massive progenitor star.Comment: Five pages, 2 figures, accepted by MNRA

    Outer boundary conditions for evolving cool white dwarfs

    White dwarf evolution is essentially a gravothermal cooling process, which,for cool white dwarfs, sensitively depends on the treatment of the outer boundary conditions. We provide detailed outer boundary conditions appropriate for computing the evolution of cool white dwarfs employing detailed non-gray model atmospheres for pure H composition. We also explore the impact on the white dwarf cooling times of different assumptions for energy transfer in the atmosphere of cool white dwarfs. Detailed non-gray model atmospheres are computed taken into account non-ideal effects in the gas equation of state and chemical equilibrium, collision-induced absorption from molecules, and the Lyman alpha quasi-molecular opacity. Our results show that the use of detailed outer boundary conditions becomes relevant for effective temperatures lower than 5800 and 6100K for sequences with 0.60 and 0.90 M_sun, respectively. Detailed model atmospheres predict ages that are up to approx 10% shorter at log L/L_sun=-4 when compared with the ages derived using Eddington-like approximations at tau_Ross=2/3. We also analyze the effects of various assumptions and physical processes of relevance in the calculation of outer boundary conditions. In particular, we find that the Ly_alpha red wing absorption does not affect substantially the evolution of white dwarfs. White dwarf cooling timescales are sensitive to the surface boundary conditions for T_eff < 6000K. Interestingly enough, non-gray effects have little consequences on these cooling times at observable luminosities. In fact, collision-induced absorption processes, which significantly affect the spectra and colors of old white dwarfs with hydrogen-rich atmospheres, have not noticeable effects in their cooling rates, except throughout the Rosseland mean opacity.Comment: 6 pages, 9 figures, to be published in Astronomy and Astrophysic
